Update DEFAULT_LOG_FORMAT to %(asctime)s [%(levelname)s] [%(name)s:%(lineno)s] %(message)s and remove DEFAULT_DATE_FORMAT so timestamps use Python's default YYYY-MM-DD HH:MM:SS,mmm, consistent with portal and publish-server. [changelog]

Move the certifi fix out of Unreleased into a 0.9.5 section and add the two merged changes (#33 log format, #34 dev-channel check) that were never recorded. The version itself comes from the v0.9.5 git tag via setuptools_scm, so the changelog is the only file to change. Co-Authored-By: Claude Sonnet 5.5 <[email protected]>
