Adds the organization default pull request template, distilled from @ahrazzle's PRs on nousresearch/hermes-agent.

Root cause

Foundation-0 has no shared PR shape: incentre-lattices carries its own result-focused template, and every other repo has nothing. New contributors guess at the format.

Changes

• pull_request_template.md (new): lede-outcome opening; Root cause with field evidence; Changes as file::symbol bullets; Validation with before/after table plus a Not covered slot; Prior art and claim label per REPO-STANDARD.md; Disclosure; Credits keeping original authorship; Left out with follow-up issue; Closes line; the human-oversight footer.
• Repos with their own template (incentre-lattices) are unaffected: a repo-level template overrides the org default.
