Add Happiness Potion logo and plugin icons - #5

Add the logo at assets/logo.png and show it at the top of the README. Give each plugin a 256px copy at assets/icon.png and point its plugin.json icon field at it, as the plugin manifest reference describes for directory listings. Regenerate the README tree.
