[Change] Code review simplification feedback loop #276

Why

The first ai_bloat capability (#269) identifies common AI-shaped bloat as advisory findings, but the evidence is still mostly single-line and local. The next step is to turn those findings into a deterministic simplification feedback loop: richer signals, intent-aware duplicate grouping, and a slash prompt that can guide safe refactors from CLI evidence.

What Changes

• Add OpenSpec change: code-review-11-simplification-feedback-loop.
• Extend specfact-code-review findings with optional simplification metadata such as confidence, rewrite_hint, canonical_pattern, intent_key, estimated_deletion_lines, and related_locations.
• Add deterministic duplicate-intent grouping across functions in the same business/domain area.
• Add --focus simplify for review runs that should feed the IDE simplification loop.
• Update /specfact.08-simplify to group by intent/domain/rule and require per-change confirmation.

Acceptance Criteria

• Review JSON remains backward-compatible and exposes optional simplification metadata.
• Duplicate-intent findings include related locations and deterministic intent keys.
• --focus simplify emits the targeted simplification queue without making findings blocking.
• /specfact.08-simplify consumes grouped evidence and still applies only accepted edits.
• Required OpenSpec validation and modules quality gates pass.
