Release 0.28.0 — the guided management surface.

Every list this interface can prune, it can now grow; and every setting whose values are knowable is chosen rather than typed. All thirty-seven configure::CATALOGUE keys resolve to a value kind and are chosen from it; unsetting one removes it so the origin column says default; /provider gained add and edit, /mcp and /plugin gained add; adding an MCP server reports whether the policy in force will let it start, naming the rule and the layer; and every verb has an argument form — io mcp, io plugin, io config — sharing one parse and one write with the guided flow.

No command was added. The palette stays at thirty-six, which matters because a group is capped at ten and occupancy leaves exactly one free slot in the product.
