Drops the built-in gh fallback rung (ADR 0012). The adapter ladder now ends at "nothing": with no project or machine adapter installed, helpers and commands skip the feature with one line naming /cdd-retrofit, and never call gh for a tracker or code-host job.

  • Helpers (cdd-worktree-done/-gc/-list/-resume) and the post-merge issue close use adapters only; the default branch still falls back to git.
  • /cdd-next-step, /cdd-pre-pr, /cdd-process-pr (repo and template copies) updated together; settings allowlist swaps gh reads for read-only adapter verbs.
  • ADR 0012 records the decision and marks the parts of ADRs 0007 and 0010 it supersedes; process doc, capability-adapters and shell-helpers docs updated.
  • Ladder, issue-close and gc gates now assert "missing adapter = skip/stop, no gh call".
  • Roadmap Phase 14 item ticked.

No issue refs were recorded for this task, so there are no close lines.

Verification: ./scripts/ci.sh — 24/24 gates pass.

The resolution ladder is now project -> machine -> nothing. With no adapter
installed, commands and helpers skip or stop with one line naming
/cdd-retrofit instead of calling gh; done keeps unconfirmed tasks, gc reaps
nothing. Gates assert missing adapter = skip/stop with no gh call.
