- prompt.md: generate a ## Map (concept → file · symbol jump-table) instead of prose file lists. - inject hook: emit hookSpecificOutput.additionalContext (plain stdout never reached the model), tiered (leaf = full map, ancestors = Purpose only), with a which-files trace log.
