docs: add coverage gates page, fix stale coverage and Dependabot docs - #416

AGENTS.md and the README said the coverage gate was >=50% on both sides, but the real gates are 70% line rate (backend) and Vitest thresholds 83/73/80/85 (frontend). Put the numbers in one new page, docs/coverage.md, and point the other docs at it so they can't drift again. Also add the page to the docs index and glossary.
