feat: tell the caller when a token revoke fails - #20
When the action token cannot be revoked, the provider now returns the warning the action token could not be revoked; it expires on its own. The broker passes it through the action response and gcb gh prints it to stderr; if the action itself failed to start, the warning still accompanies the refusal.
A runGhAction setup exception is converted to an ActionRunResult, so the revoke finally runs and the same warning path handles it.
The raw token and the revoke response body still do not cross the socket; the host log keeps the status/body detail.
The provider result uses the shared ActionRunResult type and one warning literal. Client tests run the installed CLI as a child process, so all report and count; a rejecting revoke DELETE is covered too.
This completes the Phase 2 token-lifecycle item, so its ROADMAP checkbox is now checked.
corepack yarn check: 290 pass.
Summary by CodeRabbit
New Features
Action warnings are displayed in the CLI whether an action succeeds or is refused, so callers can see non-fatal issues.
If token revocation fails, the result includes a warning that the token may remain active until it expires.
Bug Fixes
Action execution errors retain their failure details, including when token revocation also fails.
Token revocation network failures are logged while the warning is still returned.
No actionable comments were generated in the recent review.

Walkthrough
The GitHub App provider now records action execution failures and returns a warning when installation-token revocation fails. The broker forwards warnings in action responses, and the client writes them to stderr.
The provider records executor failures in the action result. If token revocation fails, it logs the failure and adds a warning. Tests cover HTTP and rejected revokes, including a setup error alongside a revoke warning. The roadmap marks caller reporting complete.
Carry warnings in action responses packages/cli/src/types.ts, packages/host/src/broker.ts, test/unit/broker.test.ts
The result and wire response types include an optional warning. The broker includes provider warnings in successful and refused responses. Tests check both outcomes.
Print warnings in the client packages/client/src/commands/gh.ts, test/unit/gh-command.test.ts
The client writes response warnings to stderr. Child-process tests check warning output and command status.
No actionable merge-blocking risk is established; the revocation warning flow is wired through provider, broker, and client layers.

Explanation
The title "feat: tell the caller when a token revoke fails" matches the main change. The PR adds a warning that the action token could not be revoked. The broker passes this warning to the caller in the action response, and gcb gh prints it to stderr. The title is specific, short, and uses the conventional commit prefix.

Approved — the caller-side half works, and nothing extra crosses the socket.
Checked against a48dc40 (e178598..a48dc40, seven files):
corepack yarn check: 284 pass; CI green on Linux, macOS, Windows (refused) and WSL.
The warning reaches the caller end to end: a static provider returning it, through the broker, into gcb gh writes warning: the action token could not be revoked; it expires on its own to stderr, leaves stdout alone, and exits 0.
Only the fixed literal crosses the socket. The revoke response-body slice (detail) still goes to log() alone, so GitHub's answer cannot ride the new path into the container.
...(result.warning === undefined ? {} : ...) is not decoration: assert.deepEqual distinguishes an own key holding undefined, so the exact-response test at test/unit/broker.test.ts:557 would fail without it, and packages/client/src/commands/pr.ts already uses the same idiom.
The warning is deliberately not in the audit record — the ROADMAP bullet asks for the host log plus the caller, so a new field would be scope.
Ticking the Token lifecycle box matches the code: mint per action, no action-token cache, finally-revoke on success, failure and timeout.
Non-blocking:
packages/host/src/providers/github-app.ts:581 — let result!: Awaited<ReturnType<typeof runGhAction>>;. The ! is unnecessary and the derived type is a step removed from the contract this method implements; let result: ActionRunResult; with a type-only import from gcb-cli typechecks unchanged. The mutable result itself is unavoidable — a return inside the try would compute the value before the finally learns that the revoke failed.
test/unit/gh-command.test.ts — the added line at packages/client/src/commands/gh.ts:130 is the only part of this PR with no test, and the file already holds the harness for it (reports truncation and timed-out writes). A provider that returns warning plus assert.match(result.stderr, /expires on its own/) is about eight lines and makes the claim self-verifying; truncation and timeout each got a client test.
github-app.ts:612 and :616 — the same literal in two branches six lines apart. A REVOKE_WARNING constant, or recording a detail per branch and logging/assigning once after the try/catch, would remove the duplication. Optional; it is small. Related: the catch branch is unexercised — the stub cannot make fetch reject today — so this is the first line there without coverage.
Observation, no change requested: when runGhAction rejects, warning is set but the return is never reached, so the caller is told only that the action failed while the host log carries the revoke detail. The ROADMAP wording ("log host-side and report") supports that, and the caller is already getting a refusal.
The reason will be displayed to describe this comment to others. Learn more.
Approved — the earlier findings are addressed.
Checked against aad2a8a:
let result: ActionRunResult; — no definite-assignment assertion, and the method's own contract type does the job. Style only: the inline type modifier on line 22 now sits next to the import type { ... } from 'gcb-cli' lines on 28–30; folding it into one of those is optional.
REVOKE_WARNING hoisted and used by both branches; the literal no longer appears twice.
test/unit/gh-command.test.ts:73 passes and is not vacuous: forcing its assertion to fail makes the --test run exit 1 with the suite reported as 1 subtest failed.
corepack yarn check: 284 pass; CI green on Linux, macOS, Windows (refused) and WSL.
The count stays at 284 although the suite gained a test, and the cause is worth recording: capture() in test/unit/gh-command.test.ts replaces process.stdout.write and restores it in finally, and under --test Node runs the file in a child that reports over stdout, so the subtests whose report lands in that window are dropped. TAP for that file is 1..3 with only ok 3 - sends a typed action ... reported, which is why the aggregate sees one test where three ran. The same shape reproduces in a three-test file where two tests patch and restore process.stdout.write: only the last is reported.
It does not hide failures — the child still exits 1 and the suite comes back ✖ … 1 subtest failed — but # tests / # fail then read fail 0, which is misleading when one of these subtests breaks. t.mock.method(process.stdout, 'write', …) kept every report in that reproduction, so it is a lead if the numbers should add up.
Unchanged and optional: the catch branch still has no coverage, because the stub cannot make fetch reject.
The reason will be displayed to describe this comment to others. Learn more.
Approved — the reporting loss is gone and the revoke catch branch is covered.
Checked against 0f2e301:
Spawning the client fixes the count, not just the symptom: TAP for test/unit/gh-command.test.ts is 1..3 with ok 1…ok 3, and corepack yarn check reports 287 pass — the previous 284, plus the two subtests that used to vanish, plus the new provider test.
The child resolves through source, not dist: with packages/*/dist absent the file still passes 3/3, so yarn test:unit works on a fresh clone and --conditions=development is doing what the comment says.
test/lib/run.ts is the right helper, and not only for the EPIPE reason: this test process serves the broker socket, so spawnSync would have deadlocked. The 5 s timeout matches the convention at test/unit/gh.test.ts:149.
The revokeReject stub path is genuinely exercised: the new test asserts both the warning and the network down log line, and the flag defaults to false, so the 500-status test and the success test are unchanged.
CI green on Linux, macOS, Windows (refused) and WSL.
One nit, no change requested: runGhCli spells the bin as packages/client/bin/gcb.js, which is relative to the cwd, while test/e2e/push.test.ts:33 resolves HELPER_BIN from import.meta.url with the comment "resolved next to this file rather than from the cwd". The same idiom here would make the file indifferent to where node --test is invoked from — two lines.
The reason will be displayed to describe this comment to others. Learn more.
Approved — the nit is fixed.
CLIENT_BIN now comes from import.meta.url, matching test/e2e/push.test.ts:33. The property it buys is real: the file passes when invoked from a directory other than the repository root, which the cwd-relative spelling could not do.
corepack yarn check: 287 pass, unchanged, as expected for a path-only change.
CI green on Linux, macOS, Windows (refused) and WSL.

Approved — a refused action no longer hides an outstanding token, and both halves are tested.
Checked against a502420:
broker.ts:300 attaches the warning to the refusal, and gh.ts:126 prints it before fail(). That order is forced by fail() ending the process, so it is the right shape rather than a duplicate of the success-path print, which stays after stdout/stderr on purpose.
Both halves are covered by new tests, not inferred: broker.test.ts "passes a warning with a failed start" (ok: false plus the warning) and gh-command.test.ts "prints a warning on a refused action" (exit 1, stderr carries both the warning and refused).
The provider composes it without extra wiring: the finally sets the warning whatever the result, so error and warning can coexist, and the refusal is not audited, so nothing new reaches the audit record.
corepack yarn check: 289 pass (287 plus the two new tests). CI green on Linux, macOS, Windows (refused) and WSL.
One doc nit: packages/cli/src/types.ts:41 still says "Set when the action succeeded but its token could not be revoked", and this commit exists because of the case where it did not. "Set when the token could not be revoked and so expires on its own" would cover both results; the WireResponse comment at :284 ("non-fatal action warning") is already neutral enough.
Observation, no change requested: a provider that throws still drops the warning — the broker's catch has no result to read, so the caller sees the generic refusal and only the host log records the revoke failure. Covering that means plumbing the warning through an error property for a path that needs an exception after the mint; result.error was the reachable one, and it is now handled.
The reason will be displayed to describe this comment to others. Learn more.
Approved — the comment now matches both results.
packages/cli/src/types.ts:41 reads "Set when the token could not be revoked and so expires on its own", which is true of a refused, timed-out or successful action, and no longer contradicts the failed-start test two files away.
corepack yarn check: 289 pass, unchanged.
CI green on Linux, macOS, Windows (refused) and WSL.

Preserve the revoke warning when runGhAction setup throws.
After the provider mints a token, runGhAction can throw from mkdtempSync or mkdirSync before its internal try. runAction still attempts revocation, but a failed revocation skips the return that adds warning. The broker then handles the thrown provider error without a warning, so gcb gh prints only the generic refusal and does not tell the caller that the token will expire on its own.
Convert this setup failure into an ActionRunResult before the existing finally. The broker's existing provider-result refusal path will then forward the warning without duplicating that logic.

Fixed in 89be815: a runGhAction setup exception becomes an ActionRunResult before the revoke finally, so a failed revoke still returns the warning with the refusal. Added a provider test for the combined error+warning case.
The reason will be displayed to describe this comment to others. Learn more.
Approved — a throw from the executor now keeps the warning, and the caller-visible message stays generic.
packages/host/src/providers/github-app.ts:594 turns a rejection into a result with error set, so the finally still revokes, the warning is attached, and the broker's existing result.error path forwards both. "Never started" is now one code path whether the executor returned that fact or threw it.
The boundary holds: the caller still sees the fixed the host could not start gh, while result.error reaches the audit record and the host log through denyAction's reason. Nothing new crosses the socket and the refusal is not recorded as an action.
The new test is machine-independent: the throw comes from buildGhArgv (pr:view's required positional against values: {}), so it passes with gh absent from PATH as well as with it there. A setup failure and a spawn failure go through the same catch, so triggering the cheaper one covers the branch.
GhRunResult.error's "could not be started at all" still reads correctly for a pre-spawn failure.
corepack yarn check: 290 pass (289 plus the new test). CI green on Linux, macOS, Windows (refused) and WSL.
Optional: the eight-line object literal restates every GhRunResult field; { ...noRun, error } with a module-level noRun would keep it from drifting if the result shape grows. Not needed for this fix.
The reason will be displayed to describe this comment to others. Learn more.
Approved — the shared no-run result is accurate and complete.
NO_RUN at packages/host/src/providers/github-app.ts:195 names the shape once, and the catch becomes { ...NO_RUN, error }. Every field ActionRunResult requires is present, so the spread is assignable without a cast, and the comment says what the value means rather than what it contains.
Single use today, but it is the right thing to share: an added field to ActionRunResult now has one place to satisfy instead of an 8-line literal to keep in sync.
corepack yarn check: 290 pass, unchanged. CI green on Linux, macOS, Windows (refused) and WSL.

Make the combined case fail during executor setup.
values: {} omits the required pr:view.number, so buildGhArgv throws before runGhAction creates or starts the executor. The provider catch is exercised, but only for argument validation. It does not cover the setup failure that this test is intended to protect.

Approved — the test now exercises the throw path it is named for.
ghPath: '\0' makes spawn() throw synchronously (ERR_INVALID_ARG_VALUE: ... must be a string without null bytes), which rejects runGhAction and lands in the provider's catch. Confirmed the error code, so this is the throw path, not the returned-error path the sibling test already covers.
values: { number: 7 } removes the argv validation as the trigger, so the case is a genuine executor rejection rather than a setup check — closer to what the catch exists for (spawn failure, EACCES, ENOSPC).
Nothing leaks on the way out: runGhAction's finally { rmSync(home, ...) } sits outside the promise, so the temp HOME is removed even though the throw happens after mkdtempSync.
corepack yarn check: 290 pass. CI green on Linux, macOS, Windows (refused) and WSL.
Optional: one line on '\0' saying that a NUL in the path is what makes spawn throw synchronously — an ordinary bad path only emits error and is returned, which is the other test — would save the next reader the lookup.

Approved — the comment says exactly the distinction that matters.
test/unit/github-app.test.ts:371 now records that a NUL makes spawn throw synchronously while an ordinary bad path is returned as result.error, which is the difference between this test and the failed-start one beside it. The '\0' literal keeps its meaning in source.
