Problem
Agent-authored tickets can hide the user problem and unresolved product decisions behind technical vocabulary and repeated work instructions.
Change
Put problem, expected outcome, limits, acceptance and open decisions before technical details in all seven contained ticket-authoring references. Align Plan, preserve task-specific restart inputs and native tracker fields, and distinguish stale-fact repair from scope changes. Add focused source regressions and a hypothetical ticket-repair scenario.

Promotion
Source PR targets develop. Main promotion requires clean exact-source verification, independent maintainer approval and a generated-only PR; do not merge develop into main.
