Fixes live-agent runs that could silently type into an unready terminal and then spend the full watchdog interval with no submitted task. Adds Codex-specific loaded-model readiness, a hard startup failure, terminal/error diagnostics in reports, traceable tests, and validated requirements coverage.\n\nVerified with unit tests, build, ESLint/Prettier, Quire validation, and a no-model Codex /status PTY check.
