hash and status. A scan (a separate, out-of-band step) is what turns "unvetted" into a
recorded verdict — the runtime never fetches and never scans.

**Scanning** (`src/skill-scan.ts` + `scripts/scan-skills.ts`, `pnpm skills:scan`) is
that step. It turns the discovered skills into the document `cisco-ai-defense/mcp-scanner`
reads (`static --resources`) and records the report as trust entries. `yara` is the
keyless detection engine — note that `prompt_defense` is _not_ an analyzer (passing it
does nothing) and that the scanner exits `0` even when it finds a threat, so the
verdict lives in the report and nowhere else. Two things the integration learned the
hard way: the scanner analyses the **URI** as well as the text, so targets carry a
skill-relative path (an absolute `/tmp/...` URI is flagged as system manipulation on
perfectly ordinary text, and it leaks the machine's layout), and a file the report never
mentions is **not** cleared — absence of a finding is not a finding of absence. A clean
scan is recorded as exactly that: a pattern scan found nothing, which is not the same
as safe.

**Delegation** (`src/delegation.ts`) is a `task` tool, off unless `--task`: a
sub-agent that runs in its own session and returns only its result. Two rules
bound it — the child never auto-approves (delegation must not amplify authority)

/**
* scripts/scan-skills.ts — scan the discovered skills and record the verdicts.
*
* pnpm skills:scan [--skills <dir>]... [--scanner <command>]
*
* Run through `tsx` (like `pnpm dev`), because it imports the loader from `src/`.
*
* Deliberately out of band. The agent never scans and never fetches at run time, so
* a run stays offline and deterministic; this writes the verdicts it *is* willing to
* consume into `$XDG_CONFIG_HOME/agent/skills.trust.json`. A flagged skill is
* recorded as blocked, and a clean one as vouched for — bounded to the content hash
* of exactly what was read.
*
* The default scanner is `cisco-ai-defense/mcp-scanner` (`uv tool install
* cisco-ai-mcp-scanner`). `yara` is the keyless detection engine; the `llm` analyzer
* catches more but wants an endpoint, and `prompt_defense` is *not* an analyzer
* (passing it silently does nothing) — it checks for missing defences, not for
* malice.
*
* Exits non-zero when anything is blocked, so it can gate a CI job.
