fix(index): retry the BM25 writer lock when a spawned process briefly holds it - #309
CI on #308 (docs only) failed in full_text_only_updates_ignore_providers_and_preserve_mode: the second incremental update could not create a BM25 writer (LockBusy).
Cause: tantivy takes its writer lock with a non-blocking flock on .tantivy-writer.lock. When another thread spawns a child process (vera-core tests run git constantly; the MCP server and vera watch also run updates in a process that spawns git), the child inherits the open lock file until it execs. The previous writer drops its handle, but the lock is still held through the child's copy, so the next writer fails at once.
Reproduced before the fix: 5 of 300 sequential insert_batch calls failed while a thread spawned true in a loop. With the fix: 0 failures across repeated runs.
Fix: one Bm25Index::writer helper replaces the five duplicated writer constructions and retries LockBusy for up to 1 s (50 × 20 ms). Vera's own index lock already serializes writers across processes, so the retry cannot mask real contention. The stress case is kept as a regression test.

Bug Fixes
Index updates and deletions now retry when the index is temporarily busy, improving reliability during concurrent operations. If the lock remains unavailable after the retries, the operation returns its result as usual.
… holds it
Tantivy takes its writer lock with a non-blocking flock. A child process
spawned by another thread inherits the lock file until it execs, so the
lock can outlive the previous writer and the next writer fails with
LockBusy. Retry for up to a second; Vera's index lock already keeps other
processes out.

BM25 writer acquisition now retries LockBusy errors up to 50 times, with a 20 ms delay between attempts. Batch insertion, bulk-build creation, deletion, and clearing use the retry helper. A test checks insertion while the writer lock is held.
A shared helper retries writer acquisition after LockBusy errors. Batch insertion, bulk-build creation, deletion, and clearing use the helper. A test verifies that insertion succeeds after the held writer lock is released.
VeraTools/vera#301: Adds git_exclude::ensure to index-lock acquisition. Both PRs address the interaction between subprocesses and the Tantivy writer lock.

Explanation
The title names the main change: the BM25 writer lock is retried when a spawned process briefly holds it. The raw summary confirms the change adds a shared writer helper that retries on LockBusy with 20 ms waits, up to 50 attempts. The objectives state the same fix. The title is specific, concise, and does not describe anything absent from the changeset.

Addressed the review: the stress test is replaced by a deterministic one that holds tantivy's writer lock on a separate handle (standing in for a child's inherited copy), releases it after 100 ms, and expects insert_batch to succeed. It fails with LockBusy when the retry is disabled and passes with it, needs no child processes, and runs on every platform. The insert_batch doc comment is back on insert_batch.
