## [Unreleased]

## [1.7.0] - 2026-10-20

### Added in 1.7.0

- Severity calibration anchors, a "Not covered" report section, guidance-vs-scan mode, and an
anti-pattern list.

### Changed in 1.7.0

- A `needs_validation` verdict now requires a non-empty `verdict_evidence.unresolved_fact`;
`validate_report.py` fails reports without it. Existing verdict files that leave it null must
be updated.

## Guidance vs Full Scan

A question about security concepts, a single finding, or how to fix something is guidance: answer
it directly and do not run the scan workflow or write files. Run the workflow below only when the
user asks to scan, audit, or check a repository. If it could mean either, ask one short question.

Report every scanner as `clean`, `findings`, `failed`, `skipped`, or `inconclusive`. Include tool

state and every displayed snippet has passed redaction.

## Anti-Patterns

- Reporting a rule match or missing best practice as a vulnerability without a reachable input
and an affected resource.
- Treating self-impact or intended same-user authority as a boundary violation.
- Assigning a severity higher than the reviewed trace supports.
- Calling a finding `confirmed` or `rejected` when a deployment or configuration fact outside
the repository decides it; use `needs_validation` with the exact `unresolved_fact`.
- Describing a scoped or partial scan as covering the whole repository.

Use exactly these scanner states: `clean`, `findings`, `failed`, `skipped`, and `inconclusive`.
`Clean` applies only to the named tool and recorded scope; it never means the application is secure.

## Severity Calibration

Scanner severity is a starting point. When assigning or adjusting `normalized_severity` for a
reviewed code finding, use these anchors and never exceed the impact the reviewed trace supports:

- `critical`: an unauthenticated actor gains code execution, full data-store access, or account
takeover.
- `high`: an actor fully defeats an explicit security control with real consequences, such as
authentication bypass, cross-tenant access, or stored script execution affecting other users.
- `medium`: a real boundary violation with limited blast radius or uncommon preconditions.
- `low`: disclosure of non-secret internals, or an effect needing sustained effort for little gain.
- `info`: a minimal-impact observation, mainly useful as part of a larger finding.

To separate `high` from `medium`, ask whether the result fully defeats a control or only weakens
it. If you cannot state the concrete damage, the severity is lower than it feels. A missing best
practice with no reachable boundary violation is not a finding.

nothing about the omitted parts, and never present the rest as secure.
