READMEs pin v5.0.0 (some module READMEs still said v4.1.0); every ops-get example fetches a subject's script. ops-get's and ops-get.ps1's own usage examples name server.sh / server.ps1, so their hashes change and the quick start has the new ones. CONTRIBUTING's naming rule describes subjects and actions; DECISIONS records the change. Refs #69
