Summary

Memory, workpad, auto review and Insights become one loop: Guardrails (docs/GUARDRAILS-SPEC.md). Nothing waits for the user any more; the project learns from the cards the user sends back, checks every delivery against what it learned, and retires what stops earning its place.

  • One store guardrails.json replaces memory.json (one-time migration at daemon start, old file renamed): command entries (run by the daemon at delivery), guideline entries (judged by the review session, injected into sessions), note entries (the former memory: decision / pitfall / contact / convention, injected only) and per-card items. States active / observing / paused / archived with caught / missed / misfire counts.
  • Learning rides on the card summarizer on rework turns: the user's own words that sent a card back become a card item; the second card sent back for the same problem promotes it to a project guideline whose scope (card types, change sizes) only widens with evidence. flowly_remember takes effect at once; no candidates, no confirmation.
  • Review on every agent delivery to In review: the project's commands, the built-in goal check, the in-scope guidelines and the card's items; a finding needs evidence (file in the diff, existing repo file, or a quote); round 2+ judges the incremental diff; failures send the card back with the user's words and [id: …] so the agent can deliver with disputes=[{id, reason}]. Three signals (caught / missed / misfire) are computed automatically and drive retirement (2 misfires in 30 days pause; 20 reviews never caught archive; commands pause after 3 unavailable runs or a missing script; notes unused 90 days archive; caps with displacement).
  • Weekly run inside Insights: re-detects whitelisted check scripts agents actually ran on ≥ 3 cards (added as observing, active after 3 reviews with a pass), applies retirement, catches up learning when card summaries are off, and may merge duplicate guidelines or notes. It never invents a guideline or a command.
  • Workpad is the acceptance list only (plus the session plan); WORKPAD_OPEN / WORKPAD_FAILING / checks / confusions / HEAD stamps are gone, SETUP_INCOMPLETE stays.
  • UI: Guardrails page (M) replaces the Memory page; a one-line review strip under the card while a review runs (Reviewing · 3/7 passed · running npm test, dot-matrix sweep, none under Reduce Motion); Insights top line from insights.guardrails; project settings keep only the Auto review switch, cost line and max rework rounds (on for new projects, 2 rounds; existing projects stay off until turned on). Tour, tip first-guardrail, What's new 1.8.0, README, AGENTS.md, i18n catalogs updated.
  • Removed: review rules editor / drafts / setup card / history analysis, flowly_propose_review_rules, the rework nudge and auto review suggestions in Insights, memory.* RPCs, reviews.detect-commands|analyze-history|analysis-status|apply-draft|discard-draft|setup-card, insights.add-review-check|pause-review-check|dismiss-review-nudge, workpads.resolve-confusion|refresh. Telemetry: f_auto_review_llm removed, c_guardrails added.

Includes the Auto Review card's phases P1–P3 (5cbb9bc) that this branch was built on. Built as spec → three Opus 5.5 phases (A foundation, B review and agent surface, C client and docs) each reviewed and reworked, then merged here.

Checks

  • npm run typecheck: exit 0 (53951db)
  • npm test: exit 0, 147 files, 1279 passed, 1 skipped (53951db)
  • npm run test:ci: exit 0 (53951db)
  • Not run on this branch: live verification on Tinylink (spec §14 items 1–6). A worktree branch cannot be installed as the flowly plugin, so it runs after the merge from the main checkout with an independent session, evidence under docs/verification/.

Known gaps (recorded in the spec §0.3)

  • Review tokens are not shown (the SDK result gives none).
  • Misfires are counted for guidelines and commands only; disputed goal / card items have no entry to count on.
  • The "new command found past the cap" Insights line has no channel yet.
  • recallStats are still recorded but nothing reads them since the Memory page is gone.

Summary by CodeRabbit

  • New Features
    • Added automatic reviews for cards delivered to In review, with progress and results on cards and in task details. Failed reviews can return cards for rework or move them to Blocked after the configured limit; reviews can also be rerun. Passing a review does not move a card to Done.
    • Added Guardrails for project guidelines, notes, and check commands. Repeated feedback can inform guidelines, and Insights tracks review and guideline activity.
    • Added settings for Auto review behavior and review models. Auto review is enabled by default for new projects and remains off for existing projects until enabled.
  • Updates
    • Workpads now focus on acceptance criteria, reviews, and plans. Project notes take effect in new sessions immediately and can be managed, filtered, sorted, and copied as project rules.
    • Added a Guardrails page, navigation shortcut, and onboarding guidance for the updated review and project-note workflows.

When a project turns on Auto review, an agent delivery to In review is
checked before it reaches the user: the workpad gate refuses failed or
stale checks, the daemon runs the project's check commands, and a
temporary review session judges the built-in "Does what you asked"
check (card plus the user's own words) and the project's LLM checks.
A failure sends the card back to its work stage with a marked rework
message; past the limit it goes to Blocked.

- Rules: command and LLM checks with type filters and pausing; drafts
from agents or the history analysis, applied by the user.
- Setup: commands detected from the repository; checks suggested from
past cards (summary of rework after delivery); Insights rework nudge,
check and pause suggestions; What's new entry for existing users.
- Spec: docs/AUTO-REVIEW-SPEC.md. Evidence, Fable's from-scratch UX
design and the unconfirmed P4 proposal under
  docs/verification/auto-review-2026-10-28/.
…o minimum length, missed needs a prior pass, phase A deviations
…nsights ledger and auto review settings (guardrails phase C)
…s, signals, weekly upkeep

- Review: shape from git numstat, items (commands active/observing, built-in goal,
in-scope guidelines, open card items), evidence filter, incremental round 2,
disputes recorded as disputed, verdict over gating items, rework message in the
user's words, results with kind; Review model else summary models.
- Signals and retirement at review end; weekly re-detect with usage evidence,
retirement, catch-up learning, Guardrails material and validated merges.
- Gate: no WORKPAD_OPEN/WORKPAD_FAILING; acknowledgeOpen only for SETUP_INCOMPLETE.
Workpad narrowed to the acceptance list; flowly_update_workpad old fields are
  WORKPAD_INVALID. autoReview narrowed to {enabled, maxRounds}.
- Removed review rules, drafts, setup card, history analysis, Insights review
suggestions and nudge, flowly_propose_review_rules and their RPCs.
- get_task without reviewRules, flowly_recall over guidelines, skill and CLI per §9.
- Telemetry: c_guardrails replaces f_auto_review_llm.
- For phase C: reviewProgress and insights.guardrails; minimal client edits to keep
typecheck green; client/phase-b-orphans.ts keeps removed strings for the catalogs.

Merge Risk: Low · up to c9eb0

The change is mergeable with minor follow-ups. Record the onboarding decision in the progress note and tighten its permission claim. Consider a single total deadline for permission-denial retries so a misbehaving model cannot hold the shared workflow queue for hours. Also clean up the earlier documentation inconsistencies and the local path in the committed verification log.

Security architecture risk: Moderate · up to c9eb0

Automatic delivery checks increase exposure to untrusted content without consistently enforcing read-only access. Permission retries can also delay checks for other projects. These are meaningful risks, although broader compromise has not been established.

Retained concerns

  • High · security · inferred: New automatic delivery reviews extend an existing default-mode security condition to review execution inside the delivered workspace. Codex receives no explicit read-only mode while processing card content, conversation, guidelines, and diffs. If its effective default permits workspace writes, malicious review input can influence execution capable of changing the work being reviewed. Denying surfaced permission requests does not restrict operations already authorized by the provider. The configuration omission predates this PR; the automatic review exposure is new, and pinned-provider authority remains unresolved.
  • Medium · reliability · observed: The permission-retry transition weakens execution-time containment by starting another full timeout after every denial round rather than consuming one overall budget. Up to 21 fifteen-minute waits can occupy a temporary session before cleanup or fallback. Automatic reviews serialize per project, while workflow analysis uses a shared chain across projects, so repeated permission cycles can delay unrelated work and retain execution resources substantially longer than intended. The 20-round cap and finally cleanup contain settled repetition but do not preserve the original time budget.
Security review details

Security Blast Radius

  • inferred — Potential review-execution integrity impact is workspace-wide, not limited to cited diff files: the review is created in the delivered card's workspace, and cards can share a working directory. Separately, workflow execution shares one queue across projects. Host credentials, other environments, and tenant boundaries cannot be bounded from the available provider evidence.

Security Findings and Attack Paths

  • inferred — The retained configuration finding applies to a new exposure path: attacker-influenced delivery content reaches an automatic review session using provider-default authority. Workspace mutation is a conditional outcome where that default permits writes without a permission request. Existing summary callers already used provider-only configuration, so the PR-specific concern is expanded autonomous exposure, not a newly removed Codex restriction. Evidence for the pinned provider's effective default remains incomplete.

Trust Boundaries and Controls

  • observed — Configured models are resolved against available providers, pending permissions receive denial responses, and shared execution accepts only idle results without an error before parsing. Review parsing restricts verdicts to supplied check identifiers, while unsupported failure evidence becomes not applicable. These controls constrain selection and result application, but do not establish a filesystem sandbox.

Resilience and Maintainability Implications

  • observed — Denial handling stops after 20 rounds, and settled model failures reach finally cleanup before fallback. Tests cover denial continuation and exhaustion. However, each round renews the full wait budget, weakening resource containment and delaying release of project or shared queues; immediate mock responses do not validate an overall deadline.

Hardening Proposals

  • proposed — Require an enforceable read-only filesystem and tool-authority configuration for every provider used by automatic reviews, and fail closed when that guarantee cannot be established. Treat textual instructions and permission-request denial as additional controls, not the containment boundary.
  • proposed — Use one absolute execution deadline across permission rounds and model fallback, with bounded termination and cleanup when the budget expires. Validate that interruption and exhausted retries release queued work rather than merely limiting the number of permission cycles.

A worktree card stores its base ref; shape and review diff start at git merge-base <baseRef> HEAD (directory and older
cards keep git diff HEAD plus the last five commits). Untracked files join the shape (text lines, binary 0) and the
review diff under # New files (untracked), without touching the index. The correction rule no longer treats
"should also" rework messages as new requirements. Findings of docs/verification/guardrails-live-2026-10-29.

A failed migration used to leave the store writable, so the first write created guardrails.json and the next start
never migrated memory.json. Writes now throw until a migration succeeds; hook writers log it with logFailure.

readHead reduces git status --porcelain to a boolean. If the review starts with uncommitted changes, a later edit can leave HEAD unchanged and dirty still true. The task and workpad revisions also remain unchanged.

The diff passed to the review session is not retained for comparison. An edit during the asynchronous review can therefore leave changed false, allowing finish to persist a verdict for code that was not reviewed. Compare a content-sensitive worktree fingerprint at completion.

🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Persist short rework feedback for weekly catch-up.

When the opening feedback is shorter than 15 characters, recordTurn drops it because it uses the default clue minimum. reworkQuote accepts the short text, but only the summary-enabled path consumes it. With summaries disabled, weekly catch-up reads only persisted feedback clues, so the correction is skipped.

Redact local home paths from the committed verification log.

The deletion RPC returns an absolute backup path. cleanup.mjs stores that path in created.json without redaction. Line 108 and the later cleanup records disclose a person's home-directory name and local filesystem layout to repository readers. Replace the committed paths with redacted examples, and redact out.backup before writing future logs. Check the committed text captures for the same path.

Validate quote provenance before accepting a failed check.

A quote that is absent from the conversation can still be valid when it appears in the card description or check-specific user words. However, the daemon does not compare quotes with any allowed source. Any nonempty invented quote passes hasEvidence, records the check as failed, and can trigger rework or eventually Blocked status.

Validate the quote against the card text, filtered conversation, and check-specific user words. Otherwise record the failure as na with reason "no evidence".

## Temporary sessions run Claude in Plan Mode — 2026-10-30

- Bug follow-up 2 (2026-10-30): the Ask SMITH card sat in Attention with nothing to do because its review ended `unavailable` ("asked for a permission") and that signal stays until the card moves. Plan Mode only helps Claude profiles; a Codex review session still stopped on its first tool call. `finishRefusingPermissions` (`server/summary-models.ts`, used by `runSummaryModels` and the workflow-insights run) now answers each pending permission request with deny and waits for the turn again, up to 20 times, so no temporary session fails on a permission any more. Tests in `server/summary-models.test.ts` (refused and continues; unrefusable and endless still fail). Unavailable still raises Attention for real failures (spec §3.2); I left that as designed. AUTO-REVIEW-SPEC §4.4, TASK-SUMMARY-SPEC §2.

Narrow the permission-handling claim.

The same entry says that unrefusable and endless requests still fail. Therefore, “no temporary session fails on a permission anymore” is too broad. State that the change handles refusable permission requests. Also write anymore as one word.

Update the claim about finishRefusingPermissions to say that it handles
refusable permission requests, without implying that all temporary sessions
avoid permission failures; retain the stated behavior for unrefusable and
endless requests. Write “anymore” as one word.

Keep one timeout deadline across permission denials.

@getpaseo/[email protected] applies timeoutMs to each waitForFinish call. WorkflowInsightsService passes 900 seconds, so the 21 waits can consume up to 5 hours 15 minutes. The total is bounded by the 20-denial cap, but timeoutMs is not a total-run deadline. Paseo returns permission as soon as the active turn needs permission, so this worst case requires each post-denial turn to approach its full timeout.

This can delay other projects because WorkflowInsightsService serializes runs through one shared chain. It does not block the separate per-project auto-review queue. Use one deadline for the entire permission loop.

Update finishRefusingPermissions to establish one deadline before the first
waitForFinish call and pass only the remaining time to subsequent waits after
permission denials. Stop the loop when no time remains, while preserving the
existing denial cap and returning the latest result.
