Subject: [PATCH] chore: remove local agent and tool ignores

A general OSS repo's gitignore does not name agent tooling. Local provisioning keeps those paths in .git/info/exclude.

- remove the .claude/, .codex/, .cursor/, .gemini/, .superpowers/, .playwright-mcp/, .mcp.json, .hyperi-ai and .kube/ ignores
- restore the .env.example exception as it was
