Add Slack/Discord notification integration

Send notifications to Slack or Discord when RepoKeeper triages an issue or reviews a PR.

Scope:

• Add optional webhook URL config for Slack/Discord
• Post formatted messages on triage events
• Include issue/PR link, classification, and summary

For the initial version, I’m thinking of keeping things simple and practical:

Platform support: Add webhook-based integration for both Slack and Discord via optional env config
Workflow: Trigger notifications for all triage events (issues and PR reviews) to start, and we can iterate on filtering if needed
Message format: Include the key details (title, link, classification, summary) with clean, platform-appropriate formatting
Structure: Implement this as a small notification service so it’s easy to extend later (e.g., additional providers or custom hooks)

This approach should keep the integration lightweight and aligned with the self-hosted nature of the project. Happy to adjust if you have a specific workflow or format in mind.

I’ve implemented a basic webhook notification flow and opened a PR.

It currently triggers after issue triage and duplicate detection using an optional WEBHOOK_URL config.

Happy to extend this further if needed.
